How do I file a claim?

We understand that sometimes there are circumstances that can make filing a claim difficult and want you to know that we are here to support you throughout this process.   • Beneficiary Claim Package:ÌýFor use by a beneficiary or group life insured who has experienced a loss. For Beneficiaries | For Employers

Can I update my beneficiaries?

You can make changes to beneficiaries at any time during the year. Reach out to the benefits administrator at your company to add or change beneficiaries on your policy.

Can I add or remove coverage for family members?

If your employer's plan includes coverage for family members, you likely only have the ability to make changes during annual enrollment or after a qualifying life status change, such as marriage, divorce or birth of a child. Reach out to the benefits administrator at your company to discuss eligibility and make changes.

What happens to my life insurance coverage if I retire or leave my company?

When you retire or leave the company, you may be eligible to continue coverage or convert to an individual policy. You can check your Certificate of Coverage found in your for details. If your coverage terminates, you will be notified of your right to convert and any other options that may be available to you.

What is Accidental Death and Dismemberment insurance?

Accidental death and dismemberment (AD&D) is a type of insurance coverage that provides a benefit in the event of a death or dismemberment caused by an accident, such as car accidents, falls, burns, and drownings.

The AD&D benefit is paid in addition to the life insurance benefit. Benefits payable due to dismemberment may cover severe accidental injuries that result in the loss of a limb (e.g., arm, leg) or the loss of a major bodily function (e.g., paralysis, sight, hearing) and may include benefits to reimburse the cost of needed modifications to one’s home and/or vehicle due to the injury, such as a wheelchair ramp or widening doors.
